I hereby authorise Thomas Wines Pty Ltd to activate me as a member in the Thomas WineClub and charge my nominated credit card with the associated cost of the wine order above, as they occur (minimum two dozen wines /12 Months, one dozen delivered in May & one dozen delivered in September).

I understand that my membership to Thomas Wines is ongoing and that I will automatically receive my wine shipment in the above stated months. I also agree that I am liable to pay for only those wines that have been despatched, prior to any given request of cancellation notified to Thomas Wines in writing.

I understand that prices may change from time to time.

I understand that for Membership benefits to apply my credit card must be valid and contact details current.

In compliance with NSW Department of Gaming and Racing I declare that I am over 18 years, and no person under 18 years will take delivery.
